Surgery of Stent for Esophageal Cancer

Surgery is suggested for the treatment of esophageal cancer

. There are two types of surgery available such as esophagectomy and Esophagogastrecotomy. The second type of surgery is recommended if the cancer is at the lower end of the esophagus or it has spread upto the stomach. Generally for this type of surgery, a part of the esophagus with cancer (probably the lower part of the esophagus) or the upper part present next to the esophagus or the lymph nodes near the esophagus is removed.

There are some cases in which all the tumors cannot be removed by surgery. In such cases the tumors start blocking the esophagus and a metallic tube called stent has to be inserted into the esophagus. This metallic stent will help the esophagus to be kept open. There are many types of stent available such as SEMS (self expandable metal stent), SEPS (self expandable plastic stent) etc.

These were used in earlier times and at present the placement of SES is the most frequently used method for palliation for esophageal cancer patients to relieve dysphagia. The patients are benefited from endoluminal implantation of SES which relieves dysphagia and obstruction thus improving patient's quality of life.

SES is carried out with a high technical success rate as a 20 minute out patient procedure. It also has a low rate of stent placements complications and is considered to be an established part of palliative treatment however serious complications may occur. The complications may be classified as in earlier stage (in less than 7 days after the procedure) which is divided as intra procedural complications, post procedure complications and late complications (7 days after the procedure).

The complications include hemorrhages, perforations, cardio respiratory, atypical chest pain, stent misplaced, stent migration, aspiration, pneumonia, esophageal reflux, recurrent food impaction, trachea-bronchial steno sis, tumor growth, fistula formation etc.

There are various types of SES that is available for the management of obstruction and ERF in patients with esophageal cancer and it is different in its physical properties and design and also depends on the manufacturer. Stent is one of the best options of treatment and it is recommended in patients with dysphagia.

The indications for self expanding stent have two categories such as relative and absolute. The absolute indications are phrenic or recurrent laryngeal nerve palsy, tracheo-esophageal fistula (in congestion with the respiratory stent) and advance esophageal cancer with metastases. The relative indications are extreme malnutrition, older age (80 or above) and esophageal stent as a bridge to surgery.

The self expanding stent are very attractive for the patients with esophageal cancer. However the stent placement is not easy to perform and is not free from complications. For the proximal stent, the respiratory symptoms and foreign body sensation are the common symptoms. For distal stent, the intractable reflux which requires the use of proton pump inhibitors is the most common symptom.
