IVF

In Vitro Fertilisation (IVF) is a process by which egg cells are fertilised by sperm outside the womb, in vitro. IVF is a major treatment in infertility when other methods of assisted reproductive technology have failed. The process involves hormonally controlling the ovulatory process, removing ova (eggs) from the woman's ovaries and letting sperm fertilise them in a fluid medium. The fertilised egg (zygote) is then transferred to the patient's uterus with the intent to establish a successful pregnancy.

IVF may be used to overcome female infertility in the woman due to problems of the fallopian tube, making fertilisation in vivo difficult. It may also assist in male infertility, where there is defect sperm quality, and in such cases intracytoplasmic sperm injection (ICSI) may be used, where a sperm cell is injected directly into the egg cell. This is used when sperm have difficulty penetrating the egg, and in these cases the partner's or a donor's sperm may be used. ICSI is also used when sperm numbers are very low. ICSI results in success rates equal to those of IVF fertilisation.

For IVF to be successful it may be easier to say that it requires healthy ova, sperm that can fertilise, and a uterus that can maintain a pregnancy. Due to the costs of the procedure, IVF is generally attempted only after less expensive options have failed.

This also avails for egg donation or surrogacy where the woman providing the egg isn't the same who will carry the pregnancy to term. This means that IVF can be used for females who have already gone through menopause. The donated oocyte can be fertilised in a crucible. If the fertilisation is successful, the zygote will be transferred into the uterus, within which it will develop into an embryo.

Who is a Candidate for IVF?

In vitro fertilization statistics suggest that couples under the age of 35 who have been trying to conceive for more than a year without results are ideal candidates for in vitro fertilization. Couples over the age of 35 who have been trying to conceive for six months may also be good candidates for IVF treatment.

The following are a few causes of infertility that may be treated with in vitro fertilization : -

Ovulation disorders

Blocked or damaged fallopian tubes

Endometriosis

Low sperm count

Sperm problems such as sperm immotility

Although there is no established age limit on in vitro fertilization, many infertility clinics and hospitals consider your age as part of the candidate screening process.

Risks and Benefits of In Vitro Fertilization

Though there are many benefits of IVF treatment, the procedure is not without certain risks. Below are general risks and benefits to consider when contemplating whether or not to undergo IVF treatment. Risks

There are certain risks associated with in vitro fertilization, as there are with any surgical procedure. Due to the technology and medical expertise necessary for IVF, cycles of treatment can be expensive, and the initial cycle does not always work. In addition, the use of multiple embryos during embryo transfer has been known to cause multiple births. Some studies suggest a slightly increased risk of genetic disorders among children conceived through in vitro fertilization, although other studies dispute these findings.

Benefits

One of the most obvious benefits of in vitro fertilization is that it can enable an infertile couple to have a baby. IVF treatment can also reduce the need for surgery on the fallopian tubes.

In a recent study, nearly 1,000 European children who were conceived through in vitro fertilization and monitored from birth until age 5 were shown to be as healthy as children who were conceived naturally.

Other assisted reproduction techniques

The following procedures have been used as alternatives to IVF but are not discussed in detail here : -

Gamete IntraFallopian Transfer (GIFT): - GIFT is similar to IVF. It is used when a woman has at least one normal Fallopian tube. Eggs are placed in this tube along with a man's sperm to fertilize there.

Zygote IntraFallopian Transfer (ZIFT): - ZIFT is tubal embryo transfer in which a woman's eggs are taken from her ovaries, fertilized in the laboratory, and put back in the fallopian tubes rather than the uterus.

Assisted fertilization techniques when not enough sperm are available or sperm quality is not sufficient to fertilize include the following : -

Partial zona dissection

Subzonal sperm injection

Intracytoplasmic sperm injection

Embryo cryopreservation (frozen fertilized egg and sperm)
