Knee Pain Treatment

Hi everyone! This is Manu Kalia, a physical therapist and Ayurveda herbalist

. In this article I wanted to answer a question asked by a lot of people about knee injuries and recovery after injuries. I want to emphasize some key concepts which would be really helpful for a lot of you guys who have had knee injury or just want to prevent knee problems.

Two things you have to keep in mind are #1 prevention, and #2, recovery after injury. Your knee is a load bearing joint. So anything that loads it, is going to aggravate it if it is injured, which means anything that involves too much walking, squatting, running, jumping, kneeling or going up and downstairs.

So first thing you have to do especially after an injury is you want to avoid all load-bearing or weight bearing activities. This is essential. Most people in my experience either under do it or overdo it. Either they are too eager to get better so they keep pushing it, which aggravates the joint even further.

The first thing you have to do is unload the joint. Which means, if you have to walk or do certain load-bearing things for function, like go to the grocery store and get what you need to get, but don't go out for a walk, or don't go out for a jog because that's going to strengthen your legs. Yes, it might strengthen the muscles but the joint itself is irritated, and if there is pain in the joint, it is going to have inhibition of the local muscles, meaning it will just shut down the local muscles and then they are not functioning and they won't support the joint as it is and then you irritate the joint further.

So the first step is to unload the joint which means you use a walker, you use crutches, you use a cane, you use a brace, you use an ACE bandage; anything to take the pressure off, or for that matter don't walk on it so much or don't load it so much.

The second thing you have to do is you have to reduce the irritability of the tissue which means that whatever activity is aggravating it, you got to stop doing it -- that's the key. So if they are static positions it does not like, meaning if I am sitting in one position for too long and then I try to get up and my knee hurts, because it gets stiff and achy, then I have to move it a little bit while I am sitting down; maybe it does not like those static positions. It gets stiff and because there is some swelling may be in the joint and it gets stiff and achy, then I need to warm it up before I get up and get moving.

The second thing could also be activity or movement oriented things. So if I move the knee too much or I do certain things and it gets irritated, maybe it does not like too much movement.

Maybe it is stasis sensitive or static position sensitive or the other way around which is it doesn't like movement, so stop doing as much movement. Or, if it doesn't like static positions you have to move it a little bit more.

The next thing you have to think about is you have to improve the circulation or flow of nutrition to that injured tissue. So in the eastern medicine way of looking at injuries, there is a blockage at a site of injury.

This means because of inflammation, dead blood cells and other waste products that have accumulated in the healing process, create almost a blockage like a dam and there is stagnation in that tissue.

So there isn't flow of either blood or for that matter Qi or Prana or energy flowing in and around that tissue or back and forth. So you have to open up that blockage while you are reducing the inflammation, and reducing pain.

Massage of the tissue and using heat around that tissue will provide some blood circulation and blood flow to the tissue. Also, taking herbal products internally; ie. certain herbs and herbal formulations, can be really helpful too because they can help reduce the inflammation and pain but not cause blockages, and help in the whole healing process.

So both externally and internally -- doing massage on the tissue and using heat to provide some circulation and blood flow in that tissue and then internally reducing the inflammation, reducing the pain and just providing or opening up flow of blood and other fluids in that tissue to provide proper nutrition to that tissue so it heals better and heals faster.
