Asking too many questions with an open end. Open ended questions require significant thinking and can easily become overly complicated.
Overusing ranking questions, or using them incorrectly. Ranking questions often have ranks from 1 to 10, but many people would prefer a 1 to 5 rank, which narrows down their choices but really still gives you the same answer.
Asking questions that have no relevance to the questionnaire. It must be very clear at all times why a question is being asked and why it is relevant to the questionnaire itself. Keep it as short as possible and try to find a way to ask multiple questions in one, by reformulating it.
Asking two questions in a single question. Again, this is very difficult. As described in point 7, it is recommended to join questions together in order to keep the overall length of the questionnaire down. However, the questions must be completely related and asked as a single question.
