Semen Analysis

A semen analysis (plural: semen analyses) evaluates certain characteristics of a male's semen and the sperm contained in the semen. It may be done while investigating a couple's infertility or after a vasectomy to verify that the procedure was successful. It is also used for testing donors for sperm donation, in stud farming and farm animal breeding.

How is it used?

A semen analysis is used to determine whether a man might be infertile-unable to get a woman pregnant. The semen analysis has many parts and tests a lot of aspects of the semen and sperm. A semen analysis to determine fertility should be performed on a minimum of two samples at least seven days apart over a period of two to three months because the sperm count and semen consistency will vary from day to day and some conditions can temporarily affect sperm levels.

The semen analysis also can be used to count sperm after a man has a vasectomy. If there are still sperm present in the semen, whether alive and still motile or apparently dead, the man and his partner will have to take precautions so that the woman does not become pregnant. He will have to return for one or more sperm counts until sperm are no longer present in his sample(s).

Tests that may be done during a semen analysis include : -

Volume. This is a measure of how much semen is present in one ejaculation.

Liquefaction time. Semen is a thick gel at the time of ejaculation and normally becomes liquid within 20 minutes after ejaculation. Liquefaction time is a measure of the time it takes for the semen to liquefy.

Sperm count. This is a count of the number of sperm present per milliliter (mL) of semen in one ejaculation.

Sperm morphology. This is a measure of the percentage of sperm that have a normal shape.

Sperm motility. This is a measure of the percentage of sperm that can move forward normally. The number of sperm that show normal forward movement in a certain amount of semen can also be measured (motile density).

pH. This is a measure of the acidity (low pH) or alkalinity (high pH) of the semen.

White blood cell count. White blood cells are not normally present in semen.

Fructose level. This is a measure of the amount of a sugar called fructose in the semen. The fructose provides energy for the sperm.

Normal Results

A few of the common normal values are listed below.

The normal volume varies from 1.5 to 5.0 milliliter per ejaculation.

The sperm count varies from 20 to 150 million sperm per milliliter.

At least 60% of the sperm should have a normal shape and show normal forward movement (motility).

However, how to interpret these values and other results from a semen analysis is not completely certain.

Normal value ranges may vary slightly among different laboratories. Talk to your doctor about the meaning of your specific test results.

What Abnormal Results Mean ?

Abnormal results may suggest a male infertility problem. For example, if the sperm count is very low or very high, there is a likelihood of being less fertile. The acidity of the semen and the presence of white blood cells (suggesting infection) may influence fertility. Testing may reveal abnormal shapes or abnormal movements of the sperm.

However, there are many unknowns in male infertility. The results from the test may fail to explain the cause. If a low sperm count or abnormal semen is found, further testing may be required.
