Esophageal Cancer and Surgery Options

Esophageal surgery is recommended when the tumor is very small and in its early stage

. If the diagnosis for esophageal cancer is made, the treatment depends on many factors depending on the patient's health condition and complications. For many people surgery is the successful method of esophageal cancer treatment. The main types of surgery for esophageal cancer are Esophagectomy (surgeon takes out a part of the esophagus) and Esophagogastrecotomy (surgeon takes out a part of esophagus and a part of upper stomach).

Esophagectomy: This is one of the main types of surgery for this cancer. This treatment is done for people with early stages of esophageal cancer and not spread much to the stomach. For this surgery, part of the esophagus containing the cancer (usually upper part upto the stomach) is removed or the lymph nodes near esophagus are removed. If squamous cell cancer is present, a larger part of the esophagus is removed during the surgery because cancer tends to start in the middle and upper part of esophagus. After taking out the parts affected with cancer in esophagus, it is connected to the stomach. A plastic tube or part of the intestine is used to join the parts together. The lymph nodes are then sent for biopsy to confirm the presence of cancer.

Esophagogastrecotomy: This type of surgery is recommended if the cancer is at the lower end of the esophagus or has spread up to the stomach. For this surgery, the part of the esophagus with cancer (usually the lower part of the esophagus) or the upper part next to the esophagus or the lymph nodes near the esophagus is removed. In some cases, all tumors cannot be removed and it starts blocking the esophagus. In such cases, a metallic tube called stent is inserted into the esophagus and this stent helps the esophagus to be kept open. Similar to previous case, the lymph nodes are sent for biopsy to confirm presence of cancer.

Both of these surgical procedures can be performed in various ways. One of the options may be minimally invasive surgery by the use of laparoscopy. In this type of surgery, only few incisions are made and operated through a small lighted telescope like device. The main goal of the esophageal cancer surgery is curative intent and palliation of symptoms.

Although both these methods are minimally invasive and traditional they are major and complications can occur. Like in any other surgery an infection, blood clot or heart attack may be possible. Also some chances are there for leakage of blood to occur in the attachment formed newly between the stomach and the esophagus. Occurance of nausea and vomiting is also possible after esophagectomy if the stomach nerves are damaged during surgery or food stays in the stomach longer. Apart from these, there may also be a difficulty in swallowing after the surgery if the esophagus starts narrowing and form strictures as it connects to stomach.
