Herpes in pregnancy is of concern due to the potential to pass the virus to the fetus during or near delivery, especially when the woman is newly infected late in pregnancy. The Society of Obstetricians and Gynecologists of Canada (SOGCs), recommend that "At delivery, women with recurrent HSV ought to be offered a Caesarean section if you will find prodromal symptoms or in the presence of a lesion suggestive of HSV."
In plain language, they recommend offering a c-section when the woman has active herpes sores or symptoms recommending one is about to appear. They explain that the chance of transmission at delivery is highest whenever a woman has a new outbreak in the third trimester, and that women should be counseled about the risk and offered a c-section. Of course this is just as suggestion, but maybe a good one. Parents to be must sit down and weigh out these scenarios and decide what is best for them and their baby.
Guidelines on the same topic from the American College of Obstetricians and Gynecologists, published in 2007, reach similar conclusions that c-section "is indicated in women with active genital lesions or prodromal symptoms," but that c-section is "not recommended for women with a history of HSV infection but no active genital disease during labor." One must remember that these are decisions that should not be taken lightly, and should be thought long and hard about.
