* The primary cause of diaper rash is irritation due to prolonged and increased exposure to wetness/moisture to the skin. If the baby's diaper is not changed frequently the skin will be touching urine and feces, urine when broken down releases ammonia which increases the PH on the skin and consequently allows enzymes present in the stool to react with the skin leading to irritation and eventual skin damage and infection.

* The actual combination of moisture and ammonia in the urine, stool, and the rubbing against the diaper material can irritate your baby's skin and lead it to look very red. If the more protected folds on the epidermis are not affected then the condition is known as Simple Irritation or Contact diaper rash. The longer wet or soiled nappies are left in contact with the baby's skin, a lot more likely nappy rash is to develop.

* Thrush infection: Candida is a fungus (a form of yeast hence the term yeast infection) it is actually the same organism that causes thrush. Yeast infection is generally only found in the skin folds creases and often around the anal region and appears in clusters or red spots around the region of the main red area, other germs (bacteria) can also infect the area leading to more severe rash. Candida infection is very common and the fungus is found everywhere in the environment. Yeast-related diaper rash occur in babies when they are not kept clean and dry as the yeast generally grows best in warm, moist places, such as under a diaper. You can differentiate a yeast infection from irritation because, unlike simple irritation, yeast causes redness in the folds and creases of a baby's skin.

* Hypersensitive reaction: The rash may occur as a reaction to diaper wipes, diapers, laundry detergent, soap, lotion, or the elastic in plastic pants. Allergic reactions are a less frequent reason for diaper rash but exist never the less. If using terry nappies, the washing up detergent or softener used in washing them might remain in the toweling, even after a full wash cycle, babies with sensitive skin may react to even minute amounts of these chemicals, especially if they contain a "biological" additive, please remember some babies' have much more sensitive skin than others and therefore reaction may differ to varying degrees.

* Butt may also develop because of the baby being on antibiotics (or if the mother is on antibiotics while breastfeeding).

* Infection: Look for yellowish, fluid-filled bumps ("pustules") and honey-colored, crusty areas, which are likely symptoms of a bacterial infection and require antibiotic treatment.

* Seborrhea (cradle cap). Sometimes a diaper rash is part of a more generalized rash, cradle cap which normally occurs on scalps can also affect other areas such as creases of the arms, legs and groin. Seborrhea usually causes the skin to look a little greasy and scaly or flaky.

* Though an infant left in a dirty diaper for too much time is more prone to develop diaper rash, any baby with sensitive skin can get a rash, even if his/her parents are diligent diaper changers.

* New foods: It is usual when infants get diaper rash when they start eating solid foods or are introduced to a new food. Any changes in the food the baby eats can affect the composition of the stool, and lead to an increase in the baby's bowel movements as well. Baby's can also develop rash as a result of reacting to something the Mother is eating.

* Outcomes of studies conducted report that the incidence of diaper rash is lower among breastfed infants- perhaps due to less acidic nature of their urine and stool and the fact they urinate less often. High-protein foods raise acid levels in urine and stool, promoting diaper rash.
