A Common Problem for Kids

Ten to 12 percent of American kids suffer from eczema. These children frequently have immediate relatives with asthma or allergies. Approximately 50 percent of kids with eczema will suffer from hay fever or allergies in the future. This fact indicates that perhaps a genetic component exists for eczema that is passed down to children from parents. Eczema is the body's natural, albeit exaggerated, response to exposure to allergens or environmental triggers, such as high heat and emotional distress. The first incidence of eczema usually occurs within the first few months of life, but rarely makes a first appearance after the child reaches five years of age. However, fully 50 percent of children who suffer from eczema will have no symptoms or occurrences by the time they reach the teen years.

Signs of an Eczema Flare-up

Eczema often first appears between the 2nd and 6th months of life. The affected area appears dry, red, and itchy. The baby may have small bumps on the cheeks and adjacent areas that become moist as they ooze clear discharge. This rash can spread to other areas of the body often to the bends of elbows and knees and backs of wrists and ankles. The oozing bumps may crust over or remain open and appear as red lesions. A child's natural reaction to scratch or rub the rash actually worsens the symptoms. Continued scratching may result in thickened skin that appears brownish in color.

Duration of Symptoms

The uncomfortable symptoms associated with eczema often improve and worsen at irregular intervals. Often, the condition will go into remission where no symptoms appear for months at a time or, in some cases, years. For most kids, the symptoms lessen in duration and frequency by the 5th or 6th year of life, but may persist into early adulthood. Others may enjoy years of remission only to have a recurrence during the onset of puberty. Eczema is a reaction to environmental irritants and allergens; it is not contagious. Parents do not need to keep a child with active eczema symptoms away from others.

Diagnosis

Many times, parents confuse eczema with other conditions, such as contact dermatitis, cradle cap (seborrheic dermatitis), and psoriasis. Each child presents with his own individual symptoms, which vary in appearance and duration, making diagnosis difficult. Since no definitive test exists to diagnose eczema, pediatricians rely heavily on clues gleaned from recording and reviewing a family history of asthma, allergies, and hay fever. The pediatrician needs to rule out other possible causes of the skin irritation or rash. He may ask about any new detergents, soaps, or lotions used on the child as well as any medications the child has taken. He will examine the affected area for indications of skin thickening where the child has scratched or rubbed the rash.

Treatment

Your local pediatrician will most often prescribe topical corticosteroids in the form of a cream or ointment. The doctor usually instructs parents to apply the steroid cream directly to the rash two times per day for up to two weeks. Since these prescription formulations come in various strengths, parents should only use corticosteroids prescribed by the physician especially for their baby. Using the incorrect strength can cause damage to the skin or other physiological problems in patients, especially babies. The doctor may prescribe oral antibiotics if a secondary infection occurred from scratching or rubbing the affected skin.
